Perscriptions
Repeat perscriptions can be ordered by either of the following methods:

• By Fax: 01 830 0489
Please allow one working day to prepare your perscription. If you would like your perscription
posted to you, please forward us a stamped addressed envelope.

Forms, Certifications etc.
If you need a doctor to complete or sign a form, please post it or hand it in to the Reception desk. Please allow one working day for it to be prepared. The isial fee is €30. For some forms eg. driving licence / Passport forms, it will be necessary to make an appointment to see the doctor. In this case the usual fee of €50 will apply.

Blood Tests
Blood can be taken, after prior consultation with the doctor. Some tests have specific requirements. eg. Fertility tests must be taken at a perticular time in a woman’s cycle.

Please always check with the Doctor.
Remember it is always your responsibility to phone for your test results.

Teaching
Medical Students may be present when you attend the surgery. If you would prefer to see the doctor with no student present, please inform reception.

Staff Safety
Abusive or disrespectful behaviour towards staff or other people attending the surgery will not be tolerated under any circumstances and such behaviour will result in immediate removal from our patient list.
